By signing, you give Moyoni permission to proceed with tattoo, piercing, skull art, and woodworking services. You understand that natural materials and body art may carry risks or imperfections, and Moyoni cannot accept liability for breakage, changes, or unexpected outcomes. Your signature confirms trust in our care, creativity, and professionalism.

Voluntary Participation, I am choosing to undergo this service of my own free will and without coercion.

Health and Safety, I confirm that I do not have any medical conditions, allergies, or infections that could interfere with the procedure or healing process. I understand that Moyoni follows strict hygiene and safety protocols, but risks such as infection, allergic reaction, or complications may still occur.

Aftercare Responsibility ,I understand that proper aftercare is essential for healing and agree to follow the instructions provided by Moyoni. Failure to do so may result in complications for which Moyoni is not responsible.

Risks and Limitations, I acknowledge that tattoos, piercings, and skull art are permanent or semi-permanent modifications, and results may vary depending on individual factors. Woodworking items may carry natural imperfections. Moyoni is not liable for dissatisfaction due to personal preference or natural variations.

Age Confirmation, I confirm that I am at least 18 years of age for tattoo and piercing services, or have parental/legal guardian consent if under 18.

Release of Liability, I release Moyoni, its artists, and affiliates from any liability for injury, loss, or damages that may arise from the services provided, except in cases of proven negligence.

I have read and understood this consent form. I have had the opportunity to ask questions and all of my concerns have been addressed. By signing below, I consent to receive the requested service(s) from Moyoni.
